Women’s participation in decision-making and equitable access to social and economic opportunities remain limited in many low-and middle-income countries. Evidence shows that only 28% of women of reproductive age in low–income countries use contraception. This persistent gap in women’s empowerment contributes to low utilization of modern contraceptives. This study aimed to determine the influence of women’s empowerment on the use of modern contraceptives among women of reproductive age attending Mbale Regional Referral Hospital. Methods This study was an analytical cross-sectional study conducted among women aged 15-49 years who attended the family planning unit at Mbale Regional Referral Hospital. A total of 326 participants was calculated using Leslie Kish’s formula. A consecutive sampling technique was used, and data were collected using a structured questionnaire. Results The proportion of women currently using modern contraception was 51.07%. The majority of the respondents were 18 - 35 years old. Injectable was the most commonly used method. Autonomy in sexual activities and decision-making was significantly associated with modern contraception (AOR = 2.75(1.32 – 5.77), P = 0.007 Conclusion Women with greater autonomy in sexual activities and participation in decision-making are more likely to use modern contraception. Educational attainment and economic participation alone may be insufficient to increase modern contraceptive use. Keywords: Autonomy in sexual activities. Educational attainment.
